Woodrow Wilson Athletics
Woodrow Wilson offers many opportunities for students to get involved in athletics. Boys tennis, girls volleyball and both boys and girls cross country are offered in the fall. Basketball seasons begin with Boys in early October, and Girls in January. We have also added Coed Wrestling, beginning in January and Coed Swimming, beginning late in February . Track and girls tennis begin in March.

Pages 1, 2, and 3 need to be uploaded to the Final Forms account. FinalForms is required for all athletes.
Please make sure that your Physical has your students full name, marked as cleared, dated and a Doctor’s signature.
New Parent Registration
How do I sign up?- Go to Vigo County Final Forms
- Locate the parent icon and click NEW ACCOUNT.
- Type your NAME, DATE OF BIRTH, and EMAIL. Next click REGISTER.
Note: You will receive an email within 2 minutes prompting you to confirm and complete your registration. If you do not receive an email, then check your spam folder. If you still cannot locate the FinalForms email, then email support@finalforms.com informing them of the issue.
- Check your email for an ACCOUNT CONFIRMATION EMAIL from FinalForms Mailman. Once received and opened, click CONFIRM YOUR ACCOUNT in the email text.
- Create your new FinalForms password. Next click CONFIRM ACCOUNT.
- Click REGISTER STUDENT for your first child.
Note: Please double check your information before you SUBMIT FORMS.
New Student Registration
What information will I need?- Basic medical history and health information.
- Insurance company and policy number.
- Doctor, dentist, and any other medical specialist contact information.
- Hospital preference and contact information.
How do I register my first student?
Important note: If you followed the steps above and registered yourself as a parent, please jump ahead to step number 3.
- Go to Vigo County Final Forms
- Click LOGIN under the parent icon.
- Locate and click ADD STUDENT button.
- Type in the LEGAL NAME and other required information. Please use your student’s SCHOOL EMAIL address, as they will receive emails from FinalForms. Then click CREATE STUDENT.
- If your student plans to participate in a sport, activity, or club, then click the checkbox for each. Then click UPDATE after making your selection. Selections may be changed until the registration deadline.
- You will need to upload a copy of your student’s Pre-participation physical. You should include the 2 page HISTORY FORM, CONSENT AND RELEASE CERTIFICATE and the PHYSICAL EXAMINATION.
- Complete each form and sign your full name (i.e. ‘Joe Jones’) in the parent signature field on EACH page. After signing each, click SUBMIT FORM and move on to the next form.
Note: Please double check your information before you SUBMIT FORMS.
- When all forms are complete you will see a “Forms Finished” message.
- Your student will have forms that they are REQUIRED to complete as well. Please have them Login and complete THEIR forms.
How do I register additional students?
Click MY STUDENTS. Then repeat steps 3 through 8 for each additional student.
How do I update information?
Login at any time and click UPDATE FORMS to update information for each student.
Returning Parents/ Students
- Go to Vigo County Final Forms
- Click LOGIN under the parent icon.
- EACH YEAR, you will need to upload your student athlete(s) physical.
- EACH YEAR, you and your student athlete(s) will need to revisit your forms.
- If things have changed, make those changes on the form, then scroll to the bottom and electronically sign EACH form.
- If nothing has changed, then scroll to the bottom and electronically sign EACH form.

Extracurricular activities when absent from school:
A student who is unable to attend school by 10:30 AM will also be unable to attend extracurricular functions later in the same day unless proper documentation (doctor’s note or legal form) is presented. In other words, you must attend the second half of the school day in order to participate in Extracurricular activities without proper documentation. Requests for special consideration should be directed to an administrator.

General Athletics Participation Information
- All potential athletes must create and complete a FinalForms account. A detailed set of instructions is available below.
- When completing your FinalForms account, please be sure to mark any sports your student/athlete may be interested in participating in throughout the school year.
- Any athlete planning to participate in 2025 – 2026 seasons will need an IHSAA Physical dated after April 1, 2025 in order to participate in the 2025 – 2026 seasons.
- Do not bring your Physical to school. It must be uploaded to FinalForms.
- Most middle school athletic teams practice 5 days a week, right after school. However, coaches can make changes to their practice schedule as needed.
- Athletes must be Academically Eligible to try out for any sport and they must remain Eligible throughout the season in order to participate. If they have more than 1.5 Fs, they may attend practice, but may not participate in/at a meet/match/game.
- Grades are checked Monday Mornings, every 2 weeks.

- The Boys Tennis team is open to any Male incoming 6th grader, 7th grader, or 8th grader.
- Please reach out to the Tennis coach, Coach McArdell with any questions.
- Athletes must be Academically Eligible to try out for any sport and they must remain Eligible throughout the season in order to participate. If they have more than 1.5 Fs, they may attend practice, but may not participate in/at a meet/match game.
- Grades are checked Monday Mornings, every 2 weeks.

Girls and Boys Cross Country
- The Cross Country teams are open to any incoming 6th grader, 7th grader, or 8th grader.
- Running is good conditioning for endurance, speed, and strengthening for all other sports.
- Most middle school athletic teams practice 5 days a week, right after school. However, coaches can make changes to their practice schedule as needed.
